REGISTER?

To register you must complete a GMS1 form to enable us to obtain your previous NHS health record.

When you register with the practice (except for children under the age of five), we will ask you to make an appointment for a health check. This will help the doctor to gain some information about your previous health and family medical history. These checks are performed by the HCA (health care assistant).
